Remember,
A child will be considered
"enrolled"
upon:
- receipt
of the application form (automatic with online form)
- receipt
of non-refundable $55 registration fee - due at Registration
- receipt
of the May 2009 (last) tuition payment - due at Registration

- The Health Care Summary MUST BE COMPLETED AND STAMPED by your doctor/clinic and returned to us
BEFORE your child begins school in September.

does not require a
physical.
Immunization Report (Shots) лл

- The State of Minnesota requires that all children entering a preschool program have an up-to-date health statement that verifies that immunizations
(MMR, D[P]T, Hib, Polio, PCV, Varicella and Hep B) are current.
